SMC Care Concern – Referral

Connect with SMC Wellness

Concerned about a student’s wellbeing at SMC? Need resources and don’t know where to go? Feeling unsure about how to reach out?
The St. Michael’s College (SMC) Care Concern Referral Form can be used to submit a concern or to connect to SMC Wellness.

SMC Wellness offers non-clinical case management support designed to support the academic success and overall wellbeing of SMC students. Our approach is collaborative and coordinated, aiming to support students with navigating student life challenges and staying connected to the resources they need.

Why would I refer a student or myself to SMC Wellness?

If you are an SMC student seeking support, or a faculty, staff or community member concerned about an SMC student, we encourage you to reach out. There are many reasons to reach out to SMC Wellness. Students who are experiencing academic or mental health challenges that are impacting the quality of their overall wellbeing can get connected with the Manager of Student Wellness (MSW).  The MSW can also support you if you are feeling concerned about a student, or uncertain about what would be the best resource for a student.

Some of the challenges that students referred to the MSW face include:

  • Financial Pressures
  • Food or Housing Insecurity
  • Personal Health and Wellness
  • Stress
  • Deteriorating Mental Health
  • Grief and Loss
  • Substance or Behavioural Concerns

What happens after I refer?

Once a referral is submitted, the MSW may reach out to you to learn more and coordinate next steps with you. The MSW works from a collaborative and student-centered approach, and as a result, participation in case management is a voluntary process. The MSW will reach out to the referred student to offer support that can translate into identifying their goals to support their holistic wellbeing. The MSW works closely and collaboratively with SMC’s Department of Residence and Student Operations, Registrar’s Office, and Campus Life; the MSW can aid students in developing individualized support plans and make direct connections to bridge students to the resources that will best serve them.

You can submit a Care Concern Referral Form to connect with the MSW and begin the support process now.
